What is Concierge Medicine?

Concierge Medicine is a type of healthcare delivery model in which patients pay a membership fee directly to their primary care physician. The fee covers a wide range of primary services including annual preventive care, acute sick care, chronic illness management, and after-hours care delivered by the physician with whom you have an established relationship. Because physicians who provide this care have fewer patients, this model allows patients more access to their doctor, with same-day or next day appointments when indicated as well as longer scheduled office visits.

Communication and Medical Records

How do I schedule an appointment?

You may schedule an appointment online for yourself (Once you become a member, we provide you with a link that permits self-scheduling).

If you prefer to talk on the phone or you need to be seen for acute problem, please call the office (206-456-2500) so that we can set up an appointment for you.

Do I have to transfer my records from my prior physician?

If I need access to records that are not available through Epic, the electronic medical record system to which I have access as a physician affiliated with Swedish, I will let you know. Once you have signed a required release of information, we can request records to which we do not have electronic access already.

Can we correspond via e-mail?

Yes! Current patients may send e-mail to care@mdseelig.com. Our communication program, Spruce Health, enables the secure exchange of e-mail, photos, and PDF attachments.

Can we correspond via text?

Yes! Current patients may send secure text messages to the office phone number, 206-456-2500. Spruce Health, the messaging program enables the secure exchange of message, photos, and PDF attachments.

Do you have access to my records?

I have access to Epic, the electronic medical record used by Swedish, Providence, UWMC, Overlake and several other major medical systems around the country.

I use Elation for my own medical record keeping. This is the same electronic medical record system used by BlissMD, Equinox Primary Care, and hundreds of other direct primary care practices.

Insurance

Do you bill Medicare, Medicaid, or other insurance plans?

As a concierge medicine practice, we do not bill any insurance. Your membership fee covers all our services in the office, over the phone, and via text, e-mail, and video visits.

Do I need health insurance if I become a member?

MDSeelig PLLC charges a monthly fee per member that includes all the covered healthcare services found in the Guide to Services https://www.mdseelig.com/guide-to-services . Your membership does not provide comprehensive health insurance coverage. It provides only the health care services specifically described. You are encouraged to obtain and maintain health insurance for services not provided by MDSeelig.

Included Services

Preventive Care

Routine health care that includes office visits, recommendations regarding appropriate screening tests, and counseling to prevent illnesses, disease, or other health problems.

Venipuncture

Venipuncture, also known as a blood draw or phlebotomy, is when someone uses a needle to take blood from a vein, usually in your arm. This is an important tool for diagnosing and monitoring many medical conditions. Examples of commonly performed tests: complete blood cell count, CBC, comprehensive metabolic panel, CMP, thyroid stimulating hormone, TSH, lipid panel.

We can draw blood for lab testing at our office, or we can send orders for you to have testing done at a lab of your choosing.

Urgent Care

Care for an illness, injury or condition that can be addressed appropriately in an outpatient office, but not so severe that it requires emergency room care.

After Hours Care

I am available 24 hours a day, seven days a week. Should an urgent need or an emergency arise, please call the office to speak with me after hours, calls will be forwarded to my cell phone. If I think that you need immediate attention, you will be directed to the nearest emergency department, and I will discuss your care with the emergency room physician. If you do not require urgent attention, I will provide advice over the phone and arrange for a follow-up visit in our office as soon as possible. If I will be unavailable, I will make arrangements for a covering physician to be available to you.

Outpatient Care Coordination

Outpatient care coordination involves deliberately sharing, retrieving, and organizing information among all the participants concerned with your care to achieve more effective and safer outcomes. This service entails helping you understand all your treatment options, including the benefits and risks, and obtaining second opinions as needed.

Chronic Disease Management

An integrated care approach to managing illness e.g., high blood pressure, high cholesterol, diabetes, arthritis, asthma, heart disease, osteoporosis which includes office visits, monitoring for disease progression and complications, tracking response to treatment and patient education. The goal is to improve your quality of life and prevent or minimize the effects of a disease.

Emergency & Inpatient Care Advocacy

Care advocacy is support for you and your family that helps and empowers you when you require care in an emergency department or hospital. Advocacy involves speaking with the physician in charge of your care and sharing your pertinent history to achieve optimal outcomes. I will help you understand proposed evaluation options, results from diagnostic testing, and treatment options.

Durable Medical Equipment (DME)

DME refers to supplies ordered by a health care clinician for everyday or extended use. I provide selected supplies, e.g., crutches, splints, and slings.

Immunizations

We provide routine vaccines for children. For adults, we provide only the standard seasonal flu vaccine.

How much is the membership fee?

The membership fee for each individual is $285 per month.

Excluded Services

Laboratory Testing

Laboratory testing is a procedure that involves testing a sample of blood, urine, or other substance from the body. Laboratory tests can help determine a diagnosis, plan treatment, check to see if treatment is working, or monitor a condition over time. Specimens collected at MDSeelig PLLC will be processed by our contracted laboratory vendor. We request a copy of your insurance information so that we can include this with the specimens that we send to the lab and the lab can bill your health insurance for the testing.

Diagnostic Imaging

Diagnostic imaging describes various methods of viewing the inside of your body to help determine the causes of an illness or injury and confirm a diagnosis. We do not provide diagnostic imaging; if it is indicated, we will refer you to the imaging center of your choice. We request a copy of your insurance information so that we can include this with the order that we send to the imaging center and the facility can bill your health insurance for the imaging.

Cost of Prescription Medications

Prescription medications are used to diagnose, cure, treat, or prevent disease; to be dispensed legally, a patient requires a medical prescription. Compared to over-the-counter drugs that can be obtained without a prescription, prescribed medications present greater risk for harm, misuse, or abuse. I will write prescriptions for necessary medications to a pharmacy of your choice. Your membership fee does not cover the cost of prescription medications. We request a copy of your insurance information so that we can include this with the order that we send to the pharmacy and the pharmacy can bill your health insurance for the medication.
